Important: Check the Orientation of Images on PACS

posted 11 Apr 2011 01:06 by Damian Wood

When you are viewing X-rays on PACS (or any other viewing system) please:
  1. consciously check the left/right markers
  2. if you magnify an X-ray to see more detail, ensure that the L/R markers remain visible on the screen.
  3. if you knowingly request, or agree to, an X-ray being taken in an unusual orientation, pass this information onto those who may review it at handover
Now that we’ve all been using PACS for a few years we will be very familiar with the usual orientation of X-rays on the screen.

It remains the responsibility of the person viewing the X-ray to ensure that they have checked the orientation before proceeding to interpret the X-ray.
Very occasionally, either due to error or an unusual protocol, or for clinical reasons an X-ray may have a non-standard orientation.

Paediatric Mock Arrests

posted 10 Mar 2011 02:59 by Damian Wood

As you have probably heard we are running ‘mock arrests’ using the sim-baby this month and then approximately once a month for the rest of the year.

The aim of these mock arrests is to use the knowledge and skills that you learn on PILS/EPLS/APLS and practice them as part of a team - or in the role of the team leader - in the most realistic setting that we can simulate. This is a multi disciplinary event for medical and nursing staff.

Please treat them as a real arrest – its surprising how realistic it feels and how it highlights areas to think about, be that roles within the team, where equipment is as well as your own knowledge and skills etc. A full cardiac arrest call is put out so it may be that you are a ward responder or find one of these situations when responding to a 2222 call.

The sim-baby is the baby version of sim-man who you will have probably met before. She is a high fidelity mannequin – she breathes, has pulses to feel, makes noises. You can cannulate her or put in an IO and give drugs and fluids. The resus trolley on the ward will be used – with the equipment etc from the trolley to ensure you are familiar with that.

You may find them stressful but they are excellent practice and I’m sure anyone who has been involved so far will have gained something from them.

We will debrief after every one and raise any learning points. If anyone feels they would like individual support after one of theses sessions we or one of the Consultants will do that too.
Thanks

IV Lorazepam - National Supply Problems

posted 7 Mar 2011 06:25 by Damian Wood

There is a national manufacturing problem with lorazepam injection (Ativan®) 4 mg/ml in the UK. There is no definite date as to when normal supplies will resume but we have been informed it may be towards the end of 2011. After discussion with the Paediatric Neurologists, we have agreed that until supply returns, IV diazepam should be substituted for lorazepam in the treatment of prolonged seizures/status epilepticus.

For more information on this change including dosing please see the Neurosciences guidelines page.
